American Diabetes Association (ADA) is a network of more than 565,000 volunteers, their families and caregivers, a professional society of nearly 16,000 health care professionals, as well as more than 250 staff members. Their mission is to prevent and cure diabetes and to improve the lives of all people affected by diabetes. ADA fulfills its mission by funding research to prevent, cure, and manage diabetes, deliver services to hundreds of communities, provide objective and credible information, and give a voice to those denied their rights because of diabetes. Continuum supports ADA’s work with their Health Equity Now Initiative that aims to address health disparities for people with diabetes. Arnold Ventures (AV) is a philanthropy dedicated to tackling some of the most pressing problems in the United States. They invest in sustainable change, building it from the ground up based on research, deep thinking, and a strong foundation of evidence. They drive public conversation, craft policy, and inspire action through education and advocacy. Continuum supports AV’s work on improving clinical trial transparency and clinical trial evidentiary standards to improve patients’ lives. Doctors for America (DFA) mobilizes doctors and medical students to be leaders in putting patients over politics on the pressing issues of the day to improve the health of our patients, communities, and nation. Continuum supports DFA’s FDA Task Force, which is working to ensure that the agency exhibits greater transparency in drug and device approvals and that patients are at the heart of every consideration. The Penn Center for Community Health Workers is a national center of excellence with the mission to advance health equity through effective, sustainable community health worker programs. Their model, IMPaCT, is a standardized, scalable program that leverages community health workers –trusted laypeople from local communities– to improve health. IMPaCT has become the most widely disseminated community health worker program in the United States. The Prevent Cancer Foundation® is the only U.S.-based nonprofit organization solely dedicated to cancer prevention and early detection. Their mission is to empower people to stay ahead of cancer through prevention and early detection. Their vision is a world where cancer is preventable, detectable, and beatable for all. Upstream partners with states to provide health centers with patient-centered, evidence-based training and technical assistance that eliminate barriers to offering the full range of contraception. The organization works directly with physicians, obstetrician-gynecologists, nurses, nurse practitioners, midwives, medical assistants and administrative staff to ensure a tailored, cohesive experience for the whole team.
